Workers' Compensation Pamphlet Support

  • Open the English or Spanish Word document and save the file to your computer. You will need Avery Labels 6879, or a comparable size (1 1/4 x 3 3/4).
  • Add your company’s current information by deleting the lines after each entry and typing in your information. Then copy and paste that information into the remaining blank label spaces.
  • Print the labels and apply to the back panel of the Workers’ Compensation pamphlet in the blank space above the line (see image below).
